Local Contact Agency

The Concho Valley Aging and Disability Resource Center serves as the Local Contact Agency (LCA).    Nursing facilities and other critical care pathways through MDS 3.0, Section Q directive identify residents who may be interested in their options for moving back into the community.    Consumers  who are Medicaid eligible are referred to Relocation Services Contractors and are connected to services through the Money Follows the Person (MFP) program.   Non medicaid eligible consumer’s are referred to the LCA for options counseling.

 

Any critical care pathway provider can refer to the LCA including:

  • Hospitals
  • Nursing Facilities

 

Appropriate referrals are individuals who: 

  • Are non medicaid eligible
  • Are using Medicare or their own resources to pay for care.
  • Are interested in options for long term care and community based support

 

When a referral is made to the LCA, a CVADRC professional  will call or visit the resident to learn what services and supports are needed through Options Counseling.

The Concho Valley ADRC serves as the Local Contact Agency for the following counties; Coke, Concho, Crockett, Irion, Kimble, Mason, Menard, McCulloch, Reagan, Runnels, Schleicher, Sterling, Sutton and Tom Green.  To Contact the Concho Valley ADRC call: 1-855-937-2372.
